Core Equipment Grant Award

**This award is to be submitted by an approve Core Facility. Applications from individual labs will not be considered.**

SUBMISSION OF APPLICATION - It is strongly encouraged for you to read over the application, collect all relevant information, and assemble all documents before starting to fill out the application. There is no "save in progress" option on this form.

Assembled document requirements: 12 pt ariel font, 1 inch margins. Applications that do not meet requirements or provide the necessary documentation will be administratively triaged.

Equipment Award Application Information

Describe the equipment and its overall use.*

Describe the equipment need for cancer related work in the core facility. *

Describe how the equipment will be maintained and the planned charge schedule to be used for it.*

If your core has received an equipment award in the past 3 years (not including the current fiscal year) provide the dates of the award(s), amount received (for each).

Provide positive outcomes foinline=true; fieldset_start=true; fieldset_label=FILE UPLOAD;r JCRC cancer research programs (i.e. contributions to publications, successful grant proposals, etc.) for the core.*

Please, upload as a single PDF the following components (in this order). All documents should be in 12 pt ARIEL font and 1 inch margins. The file should be titled "PI Last Name-Equipment-Year". Applications not meeting these requirements will not be reviewed. File size is capped at 2000 KB.

Quote for the Equipment (No page limit): If you do not have quote for the equipment, provide justification as to why not. Quote might be required before funding is awarded, if approved.

Publications from the last three-years: List of all publications from the last three years that acknowledge JCRC funding. Do not submit the actual publications, just provide links to the articles. If JCRC acknowledgment is not present in the publication, this will count against your application.

NIH Biosketches (Pi and all Co-PIs, 5 page limit for each person): Provide a current NIH biosketch for the PI and all Co-PIs.
