Faculty Travel Fellowship Application

SUBMISSION OF APPLICATION - It is strongly encouraged for you to read over the application, collect all relevant information, and assemble all documents before starting to fill out the application. There is no "save in progress" option on this form.

  • Assembled document requirements: 12 pt ariel font, 1 inch margins. Applications that do not meet requirements or provide the necessary documentation will be administratively triaged.

    Please, upload as a single PDF the following components (in this order). All documents should be in 12 pt ARIEL font and 1 inch margins. The file should be titled "PI Last Name-Year". Applications not meeting these requirements will not be reviewed. File size is capped at 2000 KB.

    Mentors, please provide the following infomration within a single PDF. Each of the categories below should be on their own page.

    Abstract: Provide the submitted and accepted abstract.

    Confirmation of Acceptance: Provide the email or letter of acceptance to the conference/workshop.

    Budget: List the amount of funding requested. Indicate the type of travel, hotelroom nights, and other costs associated with this travel.

    Mentor Information: Number of travel awards over the past three years.

    Biosketches (limit of 5 pages): Include a NIH style biosketch for the PI. All biosketchs MUST include the cancer relevant experience and why this project is relevant to the JCRC's missions in the personal statement.
