**This award is to be submitted by the PI, not the student/post-doc.**

SUBMISSION OF APPLICATION - It is strongly encouraged for you to read over the application and collection all relavant information and assemble all documents before starting to fill out the application. There is no "save in progress" option on this form.

  • Assembled document requirements: 12 pt ariel font, 1 inch margins. Applications that do not meet requirements or provide the necessary documentation will be administratively triaged.

Below are two separate sections, one the mentor needs to provide details on and the other by the student.

FILE UPLOAD

Please, upload as a single PDF the following components (in this order). All documents should be in 12 pt ARIEL font and 1 inch margins. The file should be titled "PI Last Name-Student Last Name-Year". Applications not meeting these requirements will not be reviewed. File size is capped at 2000 KB.

Mentors, please provide the following infomration within a single PDF. Each of the categories below should be on their own page.

Project Description (Limit of 1 page): This should include a hypothesis, methods, and expected outcomes. Reference are not apart of the page limit.

Publications from the last three-years: List of all publications from the last three years that acknowledge JCRC funding. Do not submit the actual publications, just provide links to the articles. 

Past CRA Information (no page limit): Outcome of awardees trained in the last three-years. List their project title, presentations, papers, and where they are now.

Student Transcripts: Unoffical transcripts are acceptable
